PHOTO/VIDEO CONSENT

I understand that during my visit(s) to Pawz Pet Cafe, photographs or videos may be taken of me. By signing this form, I hereby waive and release any claims I may have of those photos/videos. I understand that my image may be copied, edited, published or exhibited by Pawz Pet Cafe without my knowledge for marketing or otherwise. If I do not consent to these terms regarding photography/videos, I will inform Pawz Pet Cafe by selecting the appropriate drop-down response below.

COMPLIANCE WITH RULES

I have read and agree to comply with all current rules and regulations of Pawz Pet Cafe. I understand that failure to comply may result in the termination of my access to all or part of Pawz Pet Cafe.

CAT LOUNGE WAIVER AND RELEASE OF LIABILITY

By signing this form, I acknowledge and agree to the following terms:

1. Assumption of Risk: I understand that my visit to the Pawz Pet Cafe Cat Lounge involves interacting with adoptable cats, which may behave unpredictably and could bite, scratch, or otherwise cause injury. I voluntarily assume all risks associated with these interactions.

2. Waiver and Release: I waive and release Pawz Pet Cafe, its agents, employees, volunteers, insurers, and successors from any and all claims, demands, causes of action, damages, or suits of any kind, including those for personal injury, property damage, and medical expenses, related to my visit to the Cat Lounge. This waiver includes any claims arising from the negligence of Pawz Pet Cafe or any third party.

3. Indemnification: I agree not to bring any legal action against Pawz Pet Cafe for any claim released by this Agreement. If any such claim is made, I will indemnify Pawz Pet Cafe for any resulting costs, including attorney's fees and other expenses.

4. Personal Property: I understand that Pawz Pet Cafe is not responsible for any lost, stolen, or damaged valuables or property during my visit.

   
   

By signing this form, I acknowledge that I have read, understood, and agree to be bound by this Waiver and Release of Liability. I am signing this Agreement freely, voluntarily, and with full knowledge of its legal consequences.

AGE REQUIREMENT

I confirm that I am at least eighteen (18) years of age. If I am signing on behalf of a minor, I accept full responsibility for the minor’s safety and behavior, and I agree to be bound by the contents of these Terms & Conditions on their behalf.
